In a telephone directory name listing, if the person has two capital letters before the last name..?

...is it their first and middle initials or is it the first initials of the man and their partner or wife.

for ex. a person had the name as

J H Smith

would it be John H Smith as in his middle initial or like John and Hannah Smith? In a telephone directory name listing, if the person has two capital letters before the last name..?
In that case it is something like John H Smith. By default the phone account (and therefore phone book listing) is usually under one person's name.In a telephone directory name listing, if the person has two capital letters before the last name..?
Both initials belong to one Smith. If the wife or a second person to be listed would be J H Smith and M A Smith.
